Documents Required to Expedite Your Loan Application

Documents from applicant(s)
- Source of Income (i.e. four pay stubs, last two years of W-2 forms or last two years of federal income tax returns, social security award letter, pension letter, etc.)
- Mortgage Statement/Home Equity Statement
- Real Estate Tax Bill
- Water and Sewer Bill
- Homeowner’s Insurance Declaration Pages ($1,000 Max Deductible)
- Copy of valid Driver’s License
- Copy of Deed (ALL owners must be present for the in-office loan closing)
- Initial Disclosures electronic access code: The last SIX digits of your social security numbers
- If applicable, debt consolidation loan purpose, payoff letters and copies of statements

Application for membership to Pioneer Valley Credit Union is located online at www.pvcu.org under “Open Account” select eligibility through association: ACC_MA Chapter. Use your Debit/Credit card to fund $25 initial deposit with Primary Share Account and $0 deposit with Checking Account. Upon completion of membership, please let me know.
